Position Summary
The Occupational Therapist (OT) evaluates and provides skilled occupational therapy services to residents in a skilled nursing facility. The OT develops individualized treatment plans designed to improve residents' ability to perform activities of daily living, enhance functional independence, promote safety, and support the highest practicable level of physical and psychosocial well-being.
The Occupational Therapist works collaboratively with nursing, rehabilitation, physicians, families, and other members of the interdisciplinary team to provide resident-centered care in accordance with professional standards, facility policies, and applicable federal and Florida requirements.

Essential Duties and Responsibilities- Conduct comprehensive occupational therapy evaluations and reassessments to identify residents' functional abilities, limitations, and rehabilitation needs.
- Develop individualized plans of care with measurable short- and long-term goals based on evaluation findings and resident needs.
- Provide skilled occupational therapy interventions to improve or maintain functional independence.
- Address activities of daily living (ADLs), including dressing, grooming, bathing, toileting, feeding, and other self-care activities.
- Provide interventions addressing upper-extremity function, strength, coordination, balance, endurance, fine motor skills, cognition, and functional mobility as appropriate.
- Train residents in the safe and effective use of adaptive equipment and compensatory techniques.
- Evaluate residents for appropriate adaptive equipment, positioning devices, and other therapeutic needs.
- Educate residents, families, caregivers, and facility staff regarding treatment plans, safety techniques, adaptive strategies, and recommendations.
- Monitor resident progress and modify treatment plans based on clinical findings, functional changes, and established goals.
- Complete evaluations, progress notes, treatment documentation, discharge summaries, and other required clinical documentation accurately and within established timeframes.
- Participate in interdisciplinary care planning and communicate changes in resident status or treatment recommendations to appropriate team members.
- Coordinate services with Physical Therapy, Speech-Language Pathology, Nursing, Social Services, and other disciplines to support continuity of care.
- Provide appropriate supervision and direction to Occupational Therapy Assistants (COTAs) and therapy support personnel in accordance with applicable requirements and facility policy.
- Assist with discharge planning and make recommendations regarding equipment, home modifications, caregiver assistance, and continued therapy services when appropriate.
- Follow infection prevention, resident safety, emergency preparedness, and facility policies and procedures.
- Protect resident privacy and maintain confidentiality of resident information.
- Promote resident dignity, choice, independence, and quality of life.
- Report significant changes in resident condition and suspected abuse, neglect, exploitation, or safety concerns according to facility policy and applicable reporting requirements.
- Maintain productivity and caseload expectations while ensuring quality and medically necessary resident care.
- Perform other duties related to occupational therapy services as assigned.
Qualifications- Graduate of an accredited Occupational Therapy program.
- Current and active license to practice as an Occupational Therapist in the State of Florida, or otherwise legally authorized to practice as applicable.
- Current CPR/BLS certification if required by facility policy.
- Previous experience in a skilled nursing facility, long-term care, rehabilitation, geriatric, or post-acute setting preferred.
- Knowledge of occupational therapy principles, evaluation methods, treatment techniques, documentation requirements, and resident-centered care.
- Ability to communicate effectively with residents, families, healthcare professionals, and members of the interdisciplinary team.
- Ability to organize and prioritize responsibilities and manage a therapy caseload effectively.
- Must meet applicable background screening and employment requirements for the position and healthcare setting.
Knowledge, Skills, and Abilities- Strong clinical assessment and treatment-planning skills.
- Knowledge of geriatric rehabilitation and common conditions affecting the skilled nursing population.
- Ability to identify changes in resident function and modify interventions appropriately.
- Effective verbal and written communication skills.
- Strong documentation and time-management skills.
- Ability to work independently while functioning effectively as part of an interdisciplinary team.
- Compassionate and professional approach to working with residents who may have physical, cognitive, or psychosocial limitations.
- Understanding of resident rights, confidentiality, infection prevention, and safety practices.
Physical Requirements
The Occupational Therapist must be able to perform the physical functions necessary to provide safe resident care, which may include standing, walking, bending, reaching, pushing, pulling, and assisting with resident positioning, transfers, and mobility. The employee must be able to safely use therapeutic and adaptive equipment and perform other physical activities associated with occupational therapy services, with or without reasonable accommodation.

The Occupational Therapist is expected to provide services within the scope of professional practice and in accordance with applicable federal and Florida laws and regulations, professional standards, facility policies and procedures, resident rights, HIPAA requirements, infection prevention standards, and applicable Medicare/Medicaid requirements.
